This is from a group called Americans for Tax Reform about whom I know very little. The list is described as a “comprehensive” accounting of the tax raises needed to pay for “health care reform.”
Individual Mandate Excise Tax(Jan 2014): Starting in 2014, anyone not buying “qualifying” health insurance must pay an income surtax according to the higher of the following

Exemptions for religious objectors, undocumented immigrants, prisoners, those earning less than the poverty line, members of Indian tribes, and hardship cases (determined by HHS)
Note the absurdity of exempting illegal aliens as a favored class. They come to this country specifically for the health care. This is of course, the individual mandate which is unconstitutional.
